Preparing for Your First Appointment
To help make your first appointment as easy as possible, please print the following forms, fill them out, and bring them to your appointment:
In addition to the forms, please bring the following items with you:
- picture ID
- insurance cards
- list of all current medications, including over-the-counter medicines
- medical records (if you don't have them, please request them from your previous doctors)
- copy of your living will or durable power of attorney for health care (if you have one)
Please be sure to arrive 20 minutes early for your first appointment!
